DEATH CERTIFICATE FILING

Send us case vitals on your own form, or if you prefer, you may click on the appropriate form below to start entering your vitals.  Choose the generic vitals information form or click on the state in which the death occurred.  Print to pdf or printer and complete the form in it’s entirety. Email or fax back to DC filing and we’ll get started processing your death certificate.  Soon after receiving your vitals, you will receive a draft copy of your death certificate by fax or email for you to review.  Make any corrections necessary and return to us.  Your death certificate will be submitted to the certifying doctor within hours of the draft approval.  Keep checking our website under “certificate status” to following the filing process of your death certificate from origination to mailing certifieds to your funeral home.

Email or fax your death certificate vitals form to info@dcfiling.com or fax to 866-290-1259 or alternate fax 877-906-3079.  After  we receive your vitals, we will enter your death certificate into that state’s Electronic Death Registration System and generate you a draft copy to review.  Once draft is approved, we will proceed with the filing process.  If you have not received a draft copy from us within 2 hours during regular business hours, please call us immediately to make sure your fax or email was received.  Vitals received during the evenings or weekends will be addressed first thing the next business day.

Death certificate filing charge is $85 plus the cost of permits and certifieds.

KY…..All death certificates are $6.00 state wide.  There is no extra charge for a permit.  The KY provisional that you received with the body is your burial permit and as long as section D has been signed by the county coroner, the provisional is also your cremation permit.  Please fax or email a copy of the KY provisional when you submit your vitals sheet.  In case of cremation, if section D has not been signed by county coroner, we will secure that signature for you.

IN…Death certificate charges vary by county.  Costs can range from $10 to $25.  Contact us for the cost of death certificates in the county of death.  An Indiana provisional should accompany the body.  This provisional is your burial permit.  If you do not have a provisional with the body, please let us know and we will contact the facility of death or the local health department to secure you a valid provisional.  A cremation may not take place on an Indiana death until the doctor has completed the cause of death and we print an abstract from the Electronic Death Registration System which we will email or fax to you or the crematory.

OH….Death certificate charges vary by county.  Costs range from $22 to $30. Burial and cremation permits are $3 in OH.  Burial or cremation permits are generated from the Electronic Death Registration System and we will email or fax you a permit as soon as it is issued. A cremation may not take place on an Ohio death until the doctor has completed the cause of death and has sent us a copy of the signed death certificate.  Then we are able to obtain a cremation permit from the health department which we will email or fax immediately to you or the crematory so cremation may proceed.

PLEASE LET YOUR FAMILIES KNOW THAT INDIANA AND OHIO DEATHS REQUIRE WAITING FOR THE DOCTOR TO COMPLETE A DEATH CERTIFICATE BEFORE CREMATION CAN TAKE PLACE.  MEMORIAL SERVICES WHERE CREMAINS ARE DESIRED TO BE PRESENT SHOULD BE SCHEDULED FAR OUT ENOUGH TO ALLOW SUFFICIENT TIME TO PROCESS ALL NECESSARY PAPERWORK.  PLEASE CONSULT WITH OUR STAFF IN THESE CASES SO THAT WE CAN GIVE YOU AN IDEA OF TIME REQUIRED FOR COMPLETION OF THE DEATH CERTIFICATE.
